ADULTS TAJNEED (‘MEMBERSHIP’) PERSONAL DATA CONSENT FORM
FOR ADULTS 16 YEARS OR ABOVE
WHAT IS THIS CONSENT FORM FOR?
As a member of the Kerkgenootschap Ahmadiyya Moslim Djamaat (Ahmadiyya Muslim Jamaat Nederland), your consent is required to process some of your personal and sensitive (‘special category’) data for the purposes listed below (Table on page 2). This form enables you to give your consent to Kerkgenootschap Ahmadiyya Moslim Djamaat to process your personal data.
WHICH TYPES OF PERSONAL DATA WILL YOU PROCESS?
By signing this form ( below), you will provide us with your consent to process the following types of personal data:
| Identity Data | Data relating to your identity, for example your first name, last name, marital status, title, date of birth, gender and membership (AIMS) number. |
| Contact Data | Data relating to how we contact you, for example your billing address, correspondence address and email address. |
| Office-Bearer and Elections Data | Personal Data, including identity and religious data relating to: the results of internal membership election outcomes of Kerkgenootschap Ahmadiyya Moslim Djamaat and relevant appointments; and offices or positions of responsibility held in Kerkgenootschap Ahmadiyya Moslim Djamaat or any of the ‘Joint Data Controllers’ (see below). These membership elections are non-political. |
| Religious Data | Data concerning your religious beliefs and practices. For Ahmadi Muslims, this may include (if applicable) details of: when you took the initiation (Bai’at); when you signed the membership (Tajneed) form; and (if applicable) when and/or how you stopped being an Ahmadi Muslim. This is also part of your special category data. |
| Transactions/ Donations Data | This includes details of payments you make in relation to your membership of Kerkgenootschap Ahmadiyya Moslim Djamaat, including charitable donations. It may also include, in some cases, details of products you have purchased from us. |
| Special Category Data (i.e. Sensitive Data) | The EU General Data Protection Regulation 2018 defines special category data as “revealing your racial or ethnic origin, political opinions, religious or philosophical beliefs, or trade union membership […] genetic data, biometric data for the purpose of uniquely identifying a natural person, data concerning health or data concerning your sex life or sexual orientation.” |
| Children’s Data | This may include the above types of data when they relate to children (except Office-Bearer and Elections Data). This may also include details concerning education and training preferences. |
In addition, by signing this form you will provide us with your consent to process the personal data which you provided us via the enclosed Tajneed (‘Membership’) form (i.e. the data collection form to which this consent form is attached).
HOW WILL YOU COLLECT MY PERSONAL DATA?
We collect your personal data via the following means:
- From you via your direct interactions with Kerkgenootschap Ahmadiyya Moslim Djamaat (e.g. when you fill in forms or correspond with us);
- Through Kerkgenootschap Ahmadiyya Moslim Djamaat membership administration processes (e.g. via the activities of Kerkgenootschap Ahmadiyya Moslim Djamaat Departments and Office-bearers); and
- From third parties (e.g. the Joint Data Controllers mentioned below) or publicly available sources.
HOW WILL YOU USE MY PERSONAL DATA AND WHY?

WHO WILL PROCESS MY PERSONAL DATA?
In order to fulfil the Purposes mentioned below (see Table on Page 2), Kerkgenootschap Ahmadiyya Moslim Djamaat needs to process and share some of your personal data with the following Joint Data Controllers:
- Ahmadiyya Muslim Jamaat International (‘AMJ International’) (London, UK);
- AhmadiyyaMuslim Jamaat Deutschland (Germany);
- Ahmadiyya Muslim Association UK (UK);
- Tahrik Jadid Anjuman Ahmadiyya (Rabwah, Pakistan);
- Sadr Anjuman Ahmadiyya (Rabwah, Pakistan);
- Waqf-e-Jadid Anjuman Ahmadiyya (Rabwah, Pakistan);
- Sadr Anjuman Ahmadiyya (Qadian, India).
We ensure all appropriate safeguards are in place in order to transfer personal data to Joint Controllers outside of the European Economic Area (‘EEA’). In particular, AMJ International and the other Joint Controllers have adopted the standard data protection clauses published by the European Commission (see Article 46(2)(c) GDPR). WHAT ARE MY DATA PROTECTION RIGHTS?
You have the right to withdraw your consent at any time without detriment. This shall not affect the lawfulness of processing based on consent before its withdrawal. Please note that all processing of your personal data will cease once you have withdrawn consent, other than where this is required or permitted by law. Your rights are fully listed in our Privacy Notice (see above), including the rights to: object to and restrict processing of your data; Data access; rectification; erasure; and data portability.
